a group has a collective discussion and decides to adopt a consensus
decision making process with, let's say, a 2/3 majority fall back.
everyone in the collective agrees with this decision.

some people, within a group, hold meetings without letting other people
know, so that they can discuss items and take decisions about them, with a
formal consensus decision making process. more than one person leaves the
collective because (s)he feels that discussions and decision making
processes are not being inclusive and respectful of diversity. meetings
keep being organized so that it is impossible to voice any kind of dissent
regarding the decisions taken by a few people with authoritharian
tendencies.
